Job Title: Community Health Nurse Specialist

We're looking for a skilled Community Health Nurse Specialist to join our team at ChenMed. As a Community Health Nurse Specialist, you will play a vital role in providing high-quality patient care and leading our community case management team.

Key Responsibilities:
  • Provide in-home and telephonic visits to patients at high-risk for hospital admission and readmission.
  • Develop and implement comprehensive care plans for patients, including initial assessments and follow-up visits.
  • Supervise and coordinate the work of Licensed Practical Nurses (LPNs) and other team members.
  • Monitor patient progress and adjust care plans as needed.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions.
Requirements:
  • Associate degree in Nursing required; Bachelor's degree in Nursing or related field preferred.
  • Valid, active Registered Nurse (RN) license in the state of employment.
  • Minimum 2 years' clinical work experience in a hospital setting.
  • Minimum 3 years' experience in case management, home health, and community care.
