Medical Care Specialist for Seniors and Complex Needs

Abode Care Partners is a leading provider of integrated medical services, delivering quality care to individuals with complex needs in various settings. We seek an experienced Medical Care Specialist to join our team.
Responsibilities:
• Conduct comprehensive medical evaluations and develop personalized care plans
• Order diagnostic tests and collaborate with healthcare professionals to ensure high-quality patient outcomes
• Provide evidence-based medical care, including treating chronic and urgent episodic illnesses and minor injuries
• Ensure accurate and up-to-date patient medical records
Requirements:
• Active state Nurse Practitioner license and current National Board Certification
• Master's degree in Nursing (MSN) and at least five years of nursing experience, with two years of nurse practitioner experience preferred
• Strong interpersonal communication skills and proficiency in medical instruments and equipment
• Ability to engage in moderate physical activity and licensed driver with valid automobile insurance
