Job Summary

The Registered Nurse (RN) will provide care and services for a group of hospice patients, visiting patients in their homes, assisted living facilities, and hospitals. As a RN, you will complete hospice admissions and patient visits as needed. You must have great clinical and communication skills, as well as the ability to work independently and as part of a team.

Responsibilities
  • Provide nursing visits for symptom management, deaths, and admissions as needed
  • Answer calls from patients, families, and customers
  • Work closely with patients on a 1-1 level, but also receive strong support from a patient care team
  • Manage visits, documentation, meetings, and respond to urgent needs by prioritizing and triaging multiple concerns
  • Learn an electronic medical records system
  • Drive with reliable transportation, valid driver's license, and auto insurance to travel across the service area
Requirements
  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license
  • Admissions and On-call experience preferred
  • Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
