Licensed Practical Nurse

HealthCare Support is seeking a qualified Licensed Practical Nurse (LPN) to work in a Skilled Nursing Facility (SNF) setting. The ideal candidate will have experience in providing high-quality patient care and possess strong communication skills.
About the Role:
This SNF LPN position requires the ability to work collaboratively with the healthcare team to provide comprehensive care to patients. The successful candidate will be responsible for conducting patient assessments, developing care plans, and implementing interventions to promote optimal patient outcomes.
Responsibilities:
- Conduct patient assessments to identify needs and develop individualized care plans
- Implement interventions to promote patient mobility, pain management, and overall well-being
- Collaborate with the healthcare team to ensure continuity of care
- Provide education to patients and their families on disease management and self-care skills
- Maintain accurate and detailed patient records
Requirements:
- Current LPN licensure in the state
- BLS certification from the American Heart Association (AHA)
- At least one year of experience in a SNF setting
Benefits:
- Competitive hourly rate: $37.32 to $38.32
- 40 hours of work per week
- 5 shifts per week
- Traveler compensation: $1,710 gross weekly (includes weekly stipend of $1,110)
