Job Summary

We are hiring a Psychiatric Nurse Practitioner to report to our Medical Director and provide evidence-based Mental Health Care to our patients in an outpatient setting and Telehealth setting with multiple sites. This role will be an integral part of a collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.

Responsibilities:
  • Provide high-quality, patient-centered care to individuals and families in need.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination.
  • Develop and implement individualized treatment plans that address the unique needs of each patient.
Requirements:
  • Certification as a Psychiatric Nurse Practitioner (PMHNP) in New York State.
  • At least one year of experience as a Psychiatric Nurse Practitioner delivering care in a Primary Care/Ambulatory Care setting.
