About the Role

We are seeking a skilled Clinical Care Specialist to join our team at ProTouch Staffing. In this role, you will work closely with our healthcare professionals to provide exceptional care to patients in need of hospice and palliative services.

Responsibilities
  • Conduct thorough assessments of patients' needs and develop personalized care plans
  • Collaborate with interdisciplinary teams to ensure seamless transitions of care
  • Provide education and support to patients and their families on end-of-life care options
  • Document all patient interactions and care provided in accordance with regulatory requirements
  • Participate in case management and pain/symptom management initiatives
Requirements
  • Bachelor's degree in Nursing required; Master's degree preferred
  • Current Nurse Practitioner License for California
  • Minimum 1 year experience in hospice and/or palliative care
  • CPR Certification and DEA license required
