The Opportunity

We're seeking a skilled Nurse Practitioner or Physician Assistant to join our esteemed team in Aurora, CO. As a key member of our geriatric care team, you'll play a critical role in delivering comprehensive primary care to our valued patients. You will work closely with our medical director to drive practice innovation and contribute to the development of cutting-edge care models.

Your Responsibilities

  • Provide exceptional, person-centered care to our geriatric patients in an outpatient setting.
  • Collaborate with our interdisciplinary team to leverage coaching and integrated behavioral health models, promoting engagement and driving behavior change among our patients.
  • Leverage our proprietary Electronic Health Record to plan individualized care and maximize impact on our patient population.
  • Pursue professional growth through ongoing education and training, ensuring you stay at the forefront of geriatric care.
  • Contribute to a culture of innovation, teamwork, and continuous improvement within our organization.

What We Offer

We provide a comprehensive compensation package that reflects your value to our organization. The estimated salary for this role is $136,000 to $144,000 per year, commensurate with experience and qualifications.
