Job Overview

About the Role

The successful candidate will be responsible for evaluating resident needs, developing and implementing individualized care plans, and providing clinical supervision to LPNs and CNAs. Additionally, they will consult with healthcare teams to assess, plan, implement, and evaluate resident care plans. The ideal candidate will have excellent communication skills and be able to work effectively in a fast-paced environment.

Key Responsibilities
  • Evaluate residents' needs through ongoing assessment and revise care plans based on changes in their condition.
  • Provide clinical supervision to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).
  • Consult and coordinate with healthcare team members to assess, plan, implement, and evaluate resident care plans.
  • Instruct individuals and families on health education, disease prevention, and develop health improvement programs.
  • Interpret and evaluate diagnostic tests to identify and assess residents' conditions; modify treatment plans as indicated by residents' responses and conditions.
  • Monitor, record, and report symptoms and changes in residents' conditions; maintain accurate, detailed reports and records.

Qualifications and Requirements
