About the Job

  • As a Hospice Care Specialist, you will be responsible for providing high-quality care to patients with serious illnesses, focusing on comfort, dignity, and quality of life.
  • You will work closely with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.
  • Key responsibilities include assessing patient needs, developing and implementing care plans, coordinating with healthcare providers, and educating patients and families about disease management and treatment options.
  • Additional duties may include conducting home visits, monitoring patient progress, and communicating effectively with family members and healthcare professionals.

Requirements

  • A current license as a Registered Nurse in Pennsylvania is required.
  • A minimum of one year of recent, verifiable clinical experience, preferably in hospice or palliative care.
  • Prior home care experience is highly desirable but not essential.
  • Strong interpersonal skills, ability to work independently, and excellent communication skills are essential for this role.
  • Solid computer skills and prior experience with electronic medical records (EMR) are also advantageous.

Salary

The estimated salary for this position is $65,000-$85,000 per annum, based on location, experience, and qualifications.
