We are seeking a skilled Physical Therapist to join our team at Boulder Community Health. This is an excellent opportunity to work with a diverse patient population and contribute to the delivery of high-quality physical therapy services.

About the Role:

This per diem position involves working variable hours, including days, with a focus on providing quality patient care. As a Physical Therapist, you will be responsible for evaluating patients, developing individualized treatment plans, and implementing interventions to achieve optimal outcomes.

Requirements:
  • Current Colorado state license for Physical Therapy
  • BLS certification required by the end of conditional period
  • Experience in evaluating neurologically involved patients and conditions
Responsibilities:
  • Evaluating patients to determine goals and problems course of treatment
  • Selecting and establishing physical therapy diagnosis
  • Developing and maintaining individualized patient plans of care
  • Providing treatment using appropriate tools and techniques
  • Evaluating patient response to treatment and revising plans as indicated
