Position Summary:

Clinical Pharmacy Specialists collaborate with pharmacy leadership, physicians, and interdisciplinary team members to deliver pharmaceutical care in the ambulatory setting. They work in accordance with TJC, OSHA, Medicare/Medicaid standards, and Texas State Board of Pharmacy Rules and Regulations.

Key Responsibilities:

  • Collaborate with pharmacy leadership, physicians, and interdisciplinary team members to deliver pharmaceutical care in the ambulatory setting.
  • Conduct D.U.E. and patient/physician education activities as necessary.
  • Function as a source of drug information.
  • Serve as a primary preceptor for pharmacy students and residents.
  • Implement decisions set forth by the P&T Committee as written in the Hospital Formulary and comply with Health System policies and procedures.

Requirements:

  • PharmD or Bachelor's of Science degree in Pharmacy required.
  • Completion of an American Society of Health System Pharmacy PGY 1 residency or 3 years of applicable experience required.
  • Completion of an American Society of Health System Pharmacy PGY2 residency in ambulatory care preferred.
  • Must be licensed to practice pharmacy in the state of Texas.
  • Certification by the Board of Pharmaceutical Specialties in Pharmacotherapy or Ambulatory Care preferred.
