Hospice Care Specialist

We are seeking a skilled Hospice Care Specialist to join our team at Heartland Hospice. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and support to individuals and their families in need of hospice services.
Key Responsibilities:
- Perform Face-to-Face (F2F) encounters and gather clinical information to support hospice diagnosis and prognosis.
- Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
- Make recommendations to the IDG based on F2F assessment findings.
- Notify the hospice agency when unable to complete a scheduled F2F visit.
About You:
- Master's or higher degree in Nursing.
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
- Current state license as a registered nurse.
- Current state license or certificate as an advanced practice nurse.
- Current national certification as a Nurse Practitioner (ANCC or AANP).
- Must have a collaborative agreement in place.
