Clinical Case Manager for Court Collaborative Services

The Court Collaborative Program is an innovative initiative that offers mental health and substance use disorder services to adult participants in eight Alameda county treatment courts. As a Clinical Case Manager for this program, you will play a vital role in supporting participants with assessment and case management services.
Your primary goal will be to link participants with appropriate SUD and mental health services, support them with their treatment goals, and maintain communication with the court team regarding their progress. You will also lead collaborative court hearings, provide consultation and training as needed, and work closely with various stakeholders, including judges, attorneys, probation and parole officers, and representatives from the Veterans Administration and the Department of Children and Family Services.

Job Summary
We are seeking a highly skilled and compassionate Clinical Case Manager to join our team. The ideal candidate will have a Master's degree in Social Work, Psychology, or a related field, and at least two years of direct service experience in a behavioral health setting. Strong communication and interpersonal skills are essential, as well as the ability to work effectively in a fast-paced, dynamic environment.
Key Responsibilities
- Provide clinical and casework services to members served and natural supports
- Develop and implement person-centered recovery plans, collaborating with other services and agencies as needed
- Conduct psycho-social assessments and evaluations, and make referrals to emergency services as necessary
- Document all screening and assessment information in a thorough and consistent manner
- Interview clients to gather data relevant to their social and psychiatric needs, and prepare and maintain socio-psychiatric case history information
- Present case history information for review and discussion, and consult regularly with supervisors regarding problems and progress
- Attend court and staffing meetings as assigned, and prepare status reports as needed
- Update court services databases, compile statistical data, and maintain case history information and work records
- Support the management team in administrative and program activities, including data collection and analysis, research, grant preparation, and special projects
Requirements
- Master's degree in Social Work, Psychology, or a related field
- At least two years of direct service experience in a behavioral health setting
- Current registration with the Board of Behavioral Sciences in the state of employment (ASW or AMFT)
- CPR, Crisis Prevention Institute (CPI), and First Aid certification
- Valid driver's license, motor vehicle clearance, and proof of auto insurance
Benefits
We offer a competitive salary range of $34.43 - $42.52 per hour, depending on location and experience. This position requires driving and must be able to pass assault crisis/crisis prevention training. If employed in a court setting, you must be able to evade members served in the event of assaultive behavior.
