We're seeking a highly skilled Support Coordinator to join our team at Open Systems Healthcare, Inc. as a Direct Support Professional.

About the Role

As a Support Coordinator, you will play a vital role in providing high-quality support to individuals with intellectual and developmental disabilities. Your responsibilities will include:

  • Ensuring the delivery of safe and effective in-home support services.
  • Providing individualized day supports, small group day habilitation, and behavior support services that align with our policies and procedures.
  • Implementing individual support plans (ISPs) and Plans of Care to promote independence and self-advocacy.
  • Fostering positive relationships with clients, their families, and support team members.

You will also be responsible for assisting individuals with Activities of Daily Living, such as personal care, transportation, household care, and companionship. Additionally, you will help individuals acquire and/or maintain skills to achieve their ISP goals through increasing independence, self-advocacy, exploration, and integration into their community.

In this role, you will handle situations involving challenging behavior with care and professionalism and be available to address crisis situations that arise. You will also implement behavioral treatment plans and accurately document behavioral observations and progress.

The estimated salary for this position is $35,000 - $45,000 per year, depending on experience and qualifications. Benefits include medical insurance, flexible scheduling, vision insurance, weekly pay, holiday compensation, and paid orientation & training.
