Nurse Care Coordinator

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team as a Nurse Care Coordinator at our 80-bed skilled nursing community in Lake County.
This role will oversee and coordinate the development and completion of the Minimum Data Set (MDS) in accordance with current Federal and state regulations.
The successful candidate will coordinate and lead the development of a comprehensive member assessment and care plan for each member as chairperson of the community Interdisciplinary Plan of Care team.
Key responsibilities include ensuring the face validity of all MDS before electronic submission, combining timely electronic submission of all MDS, and conducting a monthly review of a random sample of the MDS to ensure accuracy and internal consistency with the medical records.

Qualifications include an active RN license in good standing, at least 2 years' experience as a Nurse, knowledge of PPS assessment and OBRA compliance, and the ability to make independent decisions and follow through with limited supervision.
