Dual Licensed Psychiatric Mental Health Nurse Practitioner

We are seeking a highly skilled and compassionate Dual Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team at Athra Systems/Crosstown Mental Health. This role involves providing psychiatric consults in a rehab setting, managing chronic care for residents in our senior living community, and conducting comprehensive psychiatric evaluations.
About the RoleThis position requires strong clinical skills in psychiatric assessment, diagnosis, and treatment, as well as chronic care management. The ideal candidate will have excellent interpersonal and communication skills, with the ability to work effectively as part of a multidisciplinary team. A compassionate and patient-centered approach to care is essential for success in this role.
Key Responsibilities- Conduct comprehensive psychiatric evaluations and provide ongoing psychiatric care and management for patients in a mental health authority setting via telehealth four days per week.
- Conduct chronic care management and behavioral health management for residents in a senior living community, focusing on the management of chronic diseases such as diabetes, hypertension, and heart disease in-person one day per week in the Baytown, TX area.
- Develop and implement individualized treatment plans for patients with mental health and substance use disorders.
- Provide medication management.
- Collaborate with a multidisciplinary team, including physicians, therapists, and social workers, to ensure coordinated and comprehensive care.
- Perform routine health assessments, diagnose and treat common health conditions, and provide preventive care.
- Educate patients and their families about chronic disease management and healthy lifestyle choices.
- Dual certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) and Family Nurse Practitioner (FNP) is mandatory.
- Master's or Doctoral degree in Nursing from an accredited institution.
- Current and valid Texas licensure as a Nurse Practitioner.
- Certification from a recognized credentialing body (e.g., ANCC, AANP).
- Minimum of two years of experience in psychiatric and family practice settings.
