Speech Language Pathologist

About the Role:
We are seeking a skilled Speech Language Pathologist to join our team at AT Home Care & Hospice. As a Speech Language Pathologist, you will play a vital role in providing direct care to patients and coordinating with the interdisciplinary team to ensure optimal care experiences.
Key Responsibilities:
- Conduct evaluations to assess speech and language function, identify equipment needs, and establish plans of care to improve patient function.
- Administer speech therapy programs, recommend communication devices and aids as needed, and maintain accurate and timely documentation.
- Participate in case conferences and ensure compliance with state and federal legal and regulatory requirements.
Requirements:
- Master's degree in Speech Language Pathology with current licensure to practice in the state of operation.
- 1+ year experience in Speech Language Pathology, with home health experience preferred.
- Commitment to clinical and documentation excellence.
