Nurse Practitioner Part-Time Position

EmpowerMe Wellness is seeking a dedicated and compassionate Nurse Practitioner to join our team in providing high-quality care to seniors in our Senior Living Communities. As a Nurse Practitioner, you will play a vital role in ensuring the health and well-being of our residents, providing medical care, education, and support to help them maintain their independence and quality of life.
Responsibilities- Provide comprehensive medical care to residents, including physical exams, diagnoses, and treatment plans.
- Develop and implement individualized care plans to address the unique needs of each resident.
- Collaborate with community health care teams to ensure seamless care transitions and coordination.
- Communicate effectively with residents, families, and healthcare professionals to ensure informed decision-making and optimal care outcomes.
- Stay up-to-date with the latest medical research, best practices, and regulatory requirements to provide evidence-based care.
- Participate in quality improvement initiatives to enhance the overall quality of care and services provided.
- Master's degree from a nationally accredited Nurse Practitioner program with full practice authority.
- Current licensure to practice professional nursing in the state of operation.
- Minimum 5 years of experience in senior living or a related field.
- Ability to prescribe medication within the scope of the state practice act.
- Excellent analytical and critical thinking skills, with a strong understanding of geriatrics and chronic illness.
- Ability to develop and maintain positive customer relationships and adapt to changing situations.
