Job SummaryThe Shelter Case Manager is responsible for coordinating with clients, utilizing motivational interviewing techniques and strength-based approaches to establish comprehensive rehousing plans, goals, and action steps aimed at promoting self-sufficiency and securing long-term, safe, affordable housing.
Key Responsibilities:- Establish regular monthly meetings with clients to review progress and provide guidance.
- Provide accurate written notices of meetings as per EOHLC Uniform Shelter Rules, Scope of Services, and program requirements.
- Identify and address barriers to housing, including credit scores, criminal records, and financial history.
- Collect and maintain vital records, financial documents, and other necessary information to complete participant files.
- Support participants in retrieving required documents if needed.
- Review client preferences and needs for tenancy, including location, roommate arrangements, and budgeting.
- Document case management efforts and services through clear, detailed, and thorough case notes.
- Provide education on affordable housing options and collaborate with other department staff.
- Makes referrals for support and services participants may need.
To be successful in this role, you will require:
- An Associate's degree in Human Services or a related field.
- At least one year of experience in providing case management services focused on pre-tenancy and tenancy-sustaining services.
- A reliable telephone and strong interpersonal skills.
- Knowledge of community resources, state, and federal benefits and services.
- Strong project management skills with the ability to administer multiple cases simultaneously.
- Bachelor math and budgeting skills.
- Ability to work effectively as a team member with other members of the program and agency leaders.
- Demonstrated prior success in case management functions or eligibility determinations for community-based social service organizations.
- Problem-solving abilities.
- Ability to communicate and collaborate with providers and community partners.
- Strong internet, database, and computer skills.
- Good documentation skills.
