Job Summary

We are seeking a highly skilled Medical Transition Coordinator to join our team at Thresholds. As a key member of our Transition Team, you will play a critical role in supporting individuals with mental illness, co-occurring substance use disorders, and physical health concerns in their transition to independent living.

Key Responsibilities
  • Assess clients' personal, medical, social, emotional, and environmental situations to plan for transition, linkage, and treatment course.
  • Provide community support, engagement activities, transition linkage, case management, and other services as needed to support clients in addressing transition needs.
  • Support clients in developing independent living skills needed to transition to the community while utilizing a strength-based and client-centered approach.
  • Collaborate with the CCMTP interdisciplinary team to identify strategies to support clients in identifying personal goals to live in a more independent setting in the community.
  • Advocate and assist clients with securing housing in a less restrictive setting in the community.
  • Provide direct assistance to help clients move, obtain household items, and explore their new community at the time of discharge.
  • Support and assist clients in preparing their new housing by shopping for furniture and essential items for independent living.
  • Transport clients to appointments as needed to prepare for discharge to the community.
  • Collaborate with the entire CCMTP team on transition planning, safety planning, clinical consultation, and problem-solving to ensure and support safe transitions to independent living.
Requirements
  • Bachelor's degree required to meet rule 132 requirements for a Mental Health Professional (MHP).
  • Demonstrated proficiency in the use of email, data reporting, record keeping, and MS Office applications.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
