We are seeking a dedicated Home Care Specialist to provide high-quality care and support to individuals with developmental disabilities in our group homes throughout Phoenix, AZ.

The Nile Health Network is a state-contracted provider of behavioral health and developmental health services. We specialize in home and community-based services, developmental group homes, ADH/CDH residential licensing, transportation, and day treatment programs/facilities. Our experienced staff is certified and trained to deliver exceptional direct care services.

Job Description

This role involves providing individualized care and support to residents in their living environments, promoting health and wellness education, and participating in ISP meetings. You will work closely with families, healthcare professionals, and other stakeholders to ensure seamless coordination of care.

A key responsibility of this position is to maintain accurate records and reports, adhere to accreditation and licensing standards, and contribute to the resolution of challenges through effective communication and collaboration.

You will be required to participate in ongoing training and professional development to enhance your skills and knowledge in areas such as HIPAA, HITECH, and first aid/CPR.

Salary and Benefits

The estimated salary for this role is $42,000-$55,000 per year, depending on experience and qualifications. In addition to competitive compensation, we offer a range of benefits, including paid time off, health insurance, and opportunities for professional growth and development.

Required Skills and Qualifications

To succeed in this role, you will need:

  • Experience working with individuals with developmental disabilities or similar populations
  • Strong communication and interpersonal skills
  • Ability to build rapport with residents, families, and healthcare professionals
  • Flexibility to work varying shifts, including evenings, weekends, and holidays
  • Basic computer skills and proficiency in electronic medical records (EMRs)
