Are you a compassionate and skilled nurse looking to make a meaningful impact in the lives of patients with hematological and oncological conditions? We invite you to join our esteemed team at Scripps Health as a Specialty Nurse Navigator.

About the Role

As a Specialty Nurse Navigator, you will play a vital role in navigating patients through the continuum of care, acting as a liaison and resource between the physician, other departments, and the patient. Your expertise will be instrumental in establishing specific, measurable, attainable, relevant, and time-specific nursing goals to guide the care planning process and promote self-reliance.

Key Responsibilities

• Navigate patients through the continuum of care, providing in-person, clinic visits as needed, and throughout transitions of care throughout all of our hematology/oncology clinic sites.
• Formulate custom care plans from assessment and clinical information, demonstrating the application of clinical guidelines, including standards of care.
• Provide disease and treatment-specific education tailored to meet patients' individual goals of care.
• Onsite rounding throughout treatment plan implementation as needed to provide needed in-person patient evaluation.
• Provide direct, hands-on nursing care, as needed.
• Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Assist patients with social services needs by identifying resources in the community and referring to these programs.
• Collaborate with system-wide authorizations team, scheduling in other departments, as needed, to ensure patients receive timely and comprehensive care.

Qualifications

• BSN plus 3 years of recent clinical nursing experience in a variety of clinical settings.
• CA RN License.
• BLS if working in a clinical setting.
• ONS Chemotherapy and Biotherapy certification.

Preferred Qualifications

• MSN.
• OCN certification if working in Hematology.
• ONN-CG certification if working in Oncology area.
• Two years of experience managing care of relevant specialized patient populations.
