Hospice RN Case Manager Job Description

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a Hospice RN Case Manager, you will be responsible for providing exceptional care to patients and their families, ensuring their physical, emotional, and spiritual needs are met.

Key Responsibilities:

  • Maintain a specified case management load.
  • Conduct initial and ongoing assessments of patients' physical and psychosocial status.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG) and healthcare providers.
  • Coordinate care among IDG members and other healthcare providers to ensure seamless delivery of services.
  • Supervise and direct services provided by LPN/LVN, Hospice Aide, and Homemaker.
  • Evaluate and coordinate the need for services in accordance with the IDG plan of care.

Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience.
  • CPR certification.
