About the Role:

We are seeking a compassionate and experienced Care Coordinator to join our team at the Massachusetts Veterans Home at Holyoke. As a Care Coordinator, you will play a pivotal role in supporting nursing staff with non-clinical and essential patient care under the direction of Registered Nurses (RNs) and Licensed Practical Nurses (LPNs).

Key Responsibilities:
  • Provides and documents bedside care to veterans based on their plan of care, taking into account their unique physical, psychosocial, emotional, and age-specific needs.
  • Ensures accurate and timely documentation of patient care activities, particularly any changes in patients' conditions, skin, behavior, or habits.
  • Maintains competencies in all aspects of patient care, nursing assistant documentation, procedures, equipment, and regulatory standards.
  • Demonstrates effective problem-solving skills by utilizing good judgment and interpersonal skills to promote a professional, safe, and productive environment.
  • Responds to emergencies and communicates effectively with other departments.
Requirements:
  • Certified Nursing Assistant (CNA) certification from an accredited program.
  • Minimum of five years of experience as a CNA in a healthcare setting, preferably at the Massachusetts Veterans Home at Holyoke or Chelsea.
  • American Heart Association-compliant CPR certification.
