We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Healthcare Professional. This is a PRN opportunity, offering flexible scheduling and the chance to make a meaningful impact in the lives of our patients.
Key Responsibilities:
- Conduct comprehensive health assessments in the comfort of patients' homes, reviewing their medical history, medication adherence, social environment, and other risk factors.
- Collaborate with healthcare providers, case managers, and other stakeholders to ensure seamless care coordination.
- Develop personalized plans to address identified health risks and promote improved health outcomes.
- Prioritize strong communication skills, verbal and written, to effectively engage with patients, families, and healthcare teams.
About the Position:
- Type: PRN-Flexible-Nurse Practitioner/Physician Assistant
- Location: Home, SNF, Telehealth, and other community environments in Wadena, Minnesota
- Hours: Days, evenings, nights, weekends (optional)
- PAY: $110.00 per completed assessment, with flexibility for 25 hours or more/month
- Benefits: PRN providers receive mileage reimbursement, no-show payment for 10 or more booked assessments/month, and limited voluntary benefits
Requirements:
- Master's degree required OR commensurate experience and satisfactory completion of NP licensure
- Current RN and NP/PA licensure in state of practice, including prescription authority or ability to obtain
- Board certification by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
- 1+ year experience as a Nurse Practitioner/Physician Assistant; ability to travel within state or out of state
- Strong computer skills, familiarity with EMR software, and excellent communication skills
Travel Requirements:
- Able to travel, valid state driver's license, and adequate automobile insurance coverage
- Matrix Provider – What to Expect:
- Make a difference in people's lives by conducting Adult/Geriatric Assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed
