Nurse Care Coordinator
About the Role:
At Medical City Dallas, we are seeking a compassionate and skilled Nurse Care Coordinator to join our team. The successful candidate will be responsible for coordinating patient care and ensuring quality outcomes. This is a challenging and rewarding role that requires strong communication and problem-solving skills.
Key Responsibilities:
- Conduct comprehensive assessments of patient needs and available resources
- Coordinate care plans and develop successful discharge plans
- Promote patient-centered care and ensure appropriate resource management
- Collaborate with the multidisciplinary team to drive effective care transitions
- Evaluate progression of care and make appropriate referrals to support patient needs
Requirements:
- Registered Nurse with current TX state license
- Graduate of an accredited nursing school, Associate's Degree required, Bachelor's Degree preferred
- 3+ years of clinical hospital nursing experience required
- 2 years of Case Management experience required
- Certification in case management preferred
