We are seeking a highly skilled and compassionate Oncology Nurse Navigator to join our team at SSM Health Saint Louis University West Pavilion. As a key member of our SLUCare Physician Group, you will play a vital role in providing exceptional patient care and navigation throughout the continuum of cancer treatment.
Job SummaryThe Oncology Nurse Navigator is responsible for coordinating efforts with physicians, advanced practice providers, and other healthcare professionals to ensure high-quality outcomes and promote quality improvement initiatives. This includes compiling data, tracking patient outcomes, and making recommendations for process enhancements.

Responsibilities and Requirements- Functions as a liaison between management staff, administration, physicians, managed care companies, community organizations, and other customers to ensure seamless communication and coordination.
- Collaborates with physicians and advanced practice providers to develop and implement individualized patient care plans that prioritize quality outcomes and patient satisfaction.
- Coordinates referrals and support for follow-up care for patients who are uninsured or underinsured, ensuring they receive the necessary resources and services.
- Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters to deliver evidence-based care.
- Assists medical providers with processing diagnostic results, highlighting significant findings and implementing appropriate interventions.

Physical DemandsThis role requires constant standing and walking, frequent lifting and carrying of objects weighing up to 25 pounds, and occasional lifting and moving of patients. You must be able to work in a fast-paced environment, think critically, and communicate effectively with diverse stakeholders.
