Care Coordination Health Partner Role Overview

The Norman Regional Health System is seeking a skilled Care Coordination Health Partner to join our team. This role is responsible for assisting in the delivery of services that address the Social Drivers of Health, referral assistance, and bridging the gap between communities, individuals, and providers.

Key Responsibilities:

  • Assist patients and their families in navigating medical services and community resources to improve population health and increase patient self-sufficiency.
  • Work closely with Care Coordinators to meet patient needs towards set goals, including hospital follow-up for inpatient/ER visits, medication compliance, coordination of services, community resources, and addressing Social Drivers of Health.
  • Support the Care Coordination departments in coordinating, procuring, and managing resources to facilitate quality care, improve patient cost outcomes, and assist in assuring a smooth transition home or to the next level of care.

Requirements:

  • Bachelor's degree in any field or 4 years of customer service experience or experience in the insurance/social work/medical fields.
  • Two years of customer service experience or experience in the insurance field/social work/medical fields.
  • Customer service experience preferred. Bilingual preferred but not required. Basic computer skills. Excellent communication both oral and written. Highly organized and self-motivated.

Benefits:

  • $18.08-$29.53/hr depending on previous work experience.

Work Schedule:
