Senior Care Professional

Job Summary:
Licensed Practical Nurse (LPN) will implement and coordinate the delivery of care in collaboration with physicians and care associates at State Street Assisted Living. The ideal candidate will exhibit a passion for serving seniors and contributing to a positive environment.
Responsibilities:
- Assessment, planning, implementation, and evaluation of resident care
- Provide high-level care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing residents' overall condition and behavior
- Assist the Director of Wellness in the development and update of resident care plans
- Maintain ongoing communication with residents, families, and physicians regarding resident care
Requirements:
- High school diploma or equivalent (GED)
- LPN License in applicable state
- First Aid & CPR Certified
- At least 1 year of related work experience
