Job Description:
We are seeking an experienced Registered Nurse to join our Home Care team as a Home Health Nurse Specialist. In this role, you will travel to patients' homes to provide personalized healthcare services, assess their conditions, administer medication, dress wounds, and coordinate care plans. Your expertise and compassion will make a significant difference in the lives of our patients and their families.
Key Responsibilities:
- Assess and chart patient observations
- Complete evaluation tasks, including reviewing medication and vital signs
- Administer Physician-prescribed medication
- Dress or redress wounds and assess healing progress
- Provide education on proper home health care procedures
- Coordinate with Physicians, Physical Therapists, and other care team members
Required Skills and Qualifications:
To be successful in this role, you must have at least 1 year of hospital experience and be registered in New York State as a Registered Professional Nurse. Your strong assessment and communication skills, attention to detail, and ability to work independently will enable you to excel in this position.
