Job Summary:

  • Coordinate care for patients across various settings, ensuring cost-effective utilization of healthcare resources.
  • Assist physicians with management and coordination of patient care.

Key Responsibilities:

  1. Develop and implement individualized care plans for patients.
  2. Collaborate with interdisciplinary teams to ensure seamless transitions of care.
  3. Conduct thorough assessments of patient needs and identify opportunities for cost savings.
  4. Analyze data to inform clinical decision-making and improve patient outcomes.
  5. Maintain accurate and detailed records of patient interactions and care plans.

Requirements:

  • Washington Registered Nurse License upon hire.
  • 5 years of clinical experience in nursing.
  • Proven experience in utilization review and case management.

Preferred Qualifications:

  • Bachelor's Degree in Nursing.
  • Certified Case Manager (CCM) certification.

Salary Information:

The estimated salary for this role is between $46.74 - $73.78 per hour, with additional compensation available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
