Salary InformationThe estimated salary for this position is between $ $85,000 and $115,000 per year, based on experience and location.
Job DescriptionWe are seeking a highly skilled Registered Nurse Case Manager to join our team at Medical City Alliance. This PRN position will involve working primarily on weekends, with hours from 8:00am-4:30pm.
As an RN Case Manager, you will be responsible for promoting patient-centered care by coordinating the plan of care for each patient's stay, managing length of stay, ensuring appropriate resource management, and developing a safe and effective discharge plan in collaboration with our multidisciplinary team.
You will work closely with patients, family members, physicians, and other healthcare professionals to ensure seamless transitions of care and optimal outcomes. Your expertise and compassion will make a significant difference in the lives of our patients and their loved ones.
This role requires strong communication and organizational skills, as well as the ability to navigate complex medical situations and provide guidance to patients and their families.
Required Skills and Qualifications- Current Texas state license as a Registered Nurse
- Graduate of an accredited nursing school (Associate's Degree required, Bachelor's Degree preferred)
- At least three years of clinical hospital nursing experience
- Two years of case management experience
- Certification in case management preferred
