End-of-Life Care Specialist

We are seeking a compassionate and skilled Hospice Registered Nurse (RN) to join our team in providing end-of-life care to patients in their homes.
Salary Range: $70,000 - $78,000 per annum
Job DescriptionAs a Hospice RN with Interim HealthCare Inc., you will have the opportunity to make a meaningful difference in the lives of our patients and their families. Your primary responsibility will be to provide high-quality, patient-centered care, focusing on symptom management, pain control, and emotional support.
Key Responsibilities:
- Conduct initial and ongoing assessments of patients' physical and psychosocial status
- Develop and implement individualized care plans in collaboration with the Interdisciplinary Group (IDG), patients, and families
- Provide education and guidance to patients and families on disease management, treatment options, and advance care planning
To succeed in this role, you must have:
Essential Qualifications:
- Graduate of an accredited Registered Nurse (RN) program
- Active, unrestricted Registered Nurse (RN) license in the State of South Carolina or compact multi-state license
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years
Preferred Qualifications:
- One (1) year of hospice care experience
