Responsibilities:
Care Coordinator – RN (Full-time)
Work collaboratively with the DON, CRM (where applicable), and various disciplines to facilitate timely completion of the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI). Provide guidance and oversight from the DON or CRM (where applicable) to ensure compliance with federal regulations and company policies. The CRS is responsible for managing the overall resident assessment process, tracking all Medicare/Managed Care/Medicaid case mix documents to ensure optimal reimbursement for services provided within the Care Center. Additional responsibilities include assisting with coordination of resident care planning processes under the direction of the DON or CRM (where applicable). Perform other duties as assigned.
This position combines full-time nursing duties with administrative responsibilities, ensuring seamless execution of tasks and duties.

Ensures regulatory compliance and Continuing Care Retirement Community operations knowledge.
Utilizes strong oral and written communication skills to maintain relationships with departments and employees in a team-oriented environment.
Proficient in Microsoft Office applications.
Sensitive to the needs of older adults and enjoys working with seniors.
The estimated salary for this position is $48,476 - $61,531 per annum, depending on location and qualifications.
