Traveling Advanced Practice Clinician for Home Visits in Kentucky

Salary and IncentivesWe are offering a base salary plus multiple incentives, including a $40,000 student loan repayment or a $25,000 sign-on bonus for individuals who have not previously participated in this program. All travel expenses are covered.
Job DescriptionThis role involves conducting in-home assessments, including evaluating past medical history, reviewing systems, reconciling medications, taking vital signs, and performing comprehensive physical exams. You will also perform evidence-based practice screenings, identify diagnoses for care management, and formulate a list of current and past medical conditions using clinical knowledge and judgment.
In addition, you will communicate findings with the member's primary care physician, educate members on disease processes, medications, and compliance, and intervene appropriately in urgent and emergent situations.
Required Skills and Qualifications- Active NP license or ability to obtain by start date
- APCs working in jurisdictions that authorize APCs to practice autonomously must have obtained approval to practice autonomously from their licensing board
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification
