About the Role:

As a skilled Physical Therapist in our Home Health team, you will play a vital role in helping patients recover from injuries, illnesses, or surgeries, and regain their independence. You will work closely with our multidisciplinary care teams to develop and implement personalized care plans that cater to each patient's unique needs.

Key Responsibilities:

  • Plan and administer prescribed physical therapy treatment and training to patients, aiming to achieve the highest level of physical function.
  • Conduct thorough assessments to evaluate patients' physical strengths and develop or pursue treatment programs, establishing measurable training objectives.
  • Collaborate with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs.
  • Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides, supervising and evaluating their performance.
  • Accurately document patients' care observations, interventions, and evaluations, ensuring timely submission for physician signature and implementation.
  • Report patients' progress to physicians, Clinical Managers, staff, patients, and families, submitting evaluation, treatment plans, progress reports, and discharge summaries as required.

Requirements:

  • Must have a degree from an accredited Physical Therapy Program approved by the APTA.
  • Minimum of one year of physical therapy experience, with Home Health experience being a plus.
  • Current and unrestricted Physical Therapy licensure.
