Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at The Manor Health and Rehabilitation Center. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our skilled nursing facility.

Responsibilities:

  • Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
  • Participate in ongoing education and training to stay current on best practices and advancements in the field of nursing.

Requirements:

  • Graduate of an approved LPN program and licensed to practice in NJ.
  • Previous experience in a skilled nursing setting preferred.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
