BAYADA Home Health Care is a leading provider of home health care services, seeking a skilled and compassionate Home Health Occupational Therapist to join our team in Lexington, Massachusetts.

About the Role

This part-time or per diem opportunity involves performing home health visits for patients in Assisted Living and Independent Living Communities in Watertown and Newton. As an integral member of our multi-disciplinary healthcare team, you will provide rehabilitative care and skilled nursing to clients, enabling them to receive the medical care required to remain at home.

As a Home Health Occupational Therapist, your key responsibilities will include:

  • Performing thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance
  • Assessing cognitive, psychological, and motor aspects of performance and developing individualized intervention plans to achieve favorable outcomes
  • Identifying equipment needs and environmental adaptations resulting in enhanced outcomes for clients
  • Continually assessing and revising the occupational therapy care plan, and participating with nursing in the multidisciplinary care plan, as appropriate
  • Educating and instructing patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living
