Responsibilities- Provide one-on-one physical therapy services to patients in their homes, tailored to meet their unique needs and goals.
- Work closely with patients, families, caregivers, and other healthcare professionals to develop and implement effective treatment plans.
- Conduct thorough assessments, identify areas of strength and weakness, and create personalized exercise programs to improve mobility, function, and overall well-being.
- Communicate regularly with patients, families, and healthcare teams to monitor progress, address concerns, and make adjustments as needed.
- Participate in ongoing education and training to stay current on best practices, research, and advancements in physical therapy.
- Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
- Current CPR certification is required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
