About the Role

As a skilled Registered Nurse (RN) at Adara Home Health, you will have the opportunity to make a meaningful impact in the lives of our clients while enjoying a fulfilling and flexible career.

Job Summary

This is a full-time position that requires the RN Case Manager to provide direct care to clients in their homes, as well as oversee and direct home health services. You will work collaboratively with clients, their families, and the healthcare team to develop and implement personalized care plans that meet their unique needs and goals.

Responsibilities
  • Provide high-quality nursing care to clients in their homes, including assessments, interventions, and evaluations
  • Develop and update client care plans in collaboration with the interdisciplinary team
  • Serve as a client advocate, ensuring their safety and well-being are addressed
  • Meet documentation standards for accuracy, timeliness, and compliance with payer requirements
  • Provide education to clients, families, caregivers, and other healthcare professionals
  • Coordinate client care with family members, caregivers, community resources, and other healthcare providers
Requirements
  • Current licensure as a Registered Nurse (RN) in the State of Minnesota
  • Proof of negative TB testing
  • Access to a dependable automobile for travel to multiple business stops
  • Valid driver's license and proof of current car insurance
  • U.S. Citizenship or valid Alien Work Permit
  • Disclosure of any conviction or criminal history records related to healthcare services
Preferred Qualifications
  • At least two years of experience in a healthcare facility or home care setting
  • Critical thinking skills, ability to follow standards/procedures, and make appropriate clinical judgments
  • Ability to work independently with minimal supervision, adapt to change, and manage multiple assignments
  • Excellent written and oral communication skills, interpersonal communication, and teaching abilities
  • Basic computer skills and willingness to learn new software programs
Estimated Salary Range

$60,000 - $80,000 per year, depending on experience and qualifications
