About the Job

This role requires expertise in Acute Care Case Management with a strong focus on UR experience, including InterQual, MCG, R1, ACM certification, adult medical UR, pediatric/maternal medical UR, and critical access hospital UR. The ideal candidate will have a proven track record of providing exceptional care and support to patients and families, with a deep understanding of the importance of timely and effective case management.

Key Responsibilities
  • Provide expert case management services to patients and families, focusing on accurate and timely assessment, planning, implementation, and evaluation of patient care needs.
  • Collaborate closely with healthcare teams to develop and implement comprehensive care plans that address the unique needs of each patient.
  • Utilize advanced knowledge of InterQual, MCG, R1, and other relevant guidelines to ensure optimal patient outcomes and cost-effective care.
  • Maintain accurate and detailed records of patient interactions, interventions, and outcomes, ensuring seamless communication and collaboration among healthcare teams.
  • Participate in regular quality improvement initiatives to enhance case management practices and promote best-in-class patient care.

Requirements
  • Bachelor's degree in Nursing or related field required; Master's degree preferred.
  • Current AK RN license and BLS certification required; ACLS/PALS certifications a plus.
  • Minimum 2-5 years of UR experience, with a strong focus on InterQual, MCG, R1, and ACM certification.
  • Advanced knowledge of Microsoft Office/Teams, Epic EMR system, and other relevant software applications.
  • Excellent communication, interpersonal, and problem-solving skills, with a customer-centric approach to patient care.
