Nurse Case Manager Position

As a Registered Nurse Case Manager at Summa Health at Home and Hospice, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. This position requires a compassionate and skilled nurse who can assess patient conditions, develop individualized care plans, and provide clinical care under the direction of a physician.
Key Responsibilities:
• Observe and monitor patient conditions, performing OASIS assessments to develop and adjust care plans as needed.
• Administer medication as prescribed by the physician and prioritize visits for high-risk patients to minimize risk.
• Oversee and supervise the total care of patients provided by nurse aides and LPNs.
• Manage multi-disciplinary care, promoting continuity of care with appropriate admissions, transfers, and discharges.
Requirements:
• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting, with home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
