We are seeking a skilled Traveling Healthcare Professional to join our team at Optum Home & Community Care. This is a 100% travel role that requires willingness for travel within the states of Ohio, Wisconsin, Oregon, and Washington.

About the Role:

This position involves providing clinical coverage for patients in assisted living facilities, independent senior living facilities, and home settings. As a member of our Senior Community Care (SCC) team, you will work with an interdisciplinary care team to coordinate the delivery and provision of clinical care to our highest-risk health plan members.

Responsibilities:

  • Provide comprehensive assessments and manage acute medical situations or exacerbations of chronic conditions.
  • Effectively manage acute and chronic medical and behavioral conditions.
  • Communicate and collaborate with the interdisciplinary care team.
  • Ensure accurate and complete documentation, including ICD-10 conditions.

Requirements:

  • Active NP or PA license or ability to obtain by start date.
  • For NPs: Active ANCC or AANP national certification in Family, Adult, Adult-Gerontology Primary Care, etc.
  • Current, active DEA licensure/prescriptive authority or eligibility to obtain post-hire.
  • 1+ years of clinical/family/geriatric practice experience as an NP or PA.
  • Ability to travel via car, airplane, etc., and obtain additional licensure in assigned geographic areas.

Preferred Qualifications:

  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
