Job Summary

Edenbridge Health is seeking a highly skilled and experienced Center Manager to lead our PACE program at Skyland. The successful candidate will be responsible for building and leading the PACE program, applying PACE fundamentals to our innovative care approach, and working autonomously with the support of expert PACE operators, geriatricians, and project managers.

Main Responsibilities

Facilitate daily morning IDT meetings
Participate in QI committees and activities as assigned and requested
Manage participant and caregiver/family grievance process, including taking the initial report and ensuring that grievances are addressed and documented in accordance with CMS and DHCF requirements
Supervise Transportation Manager, Recreational Therapist, and MSW-level Social Workers
Provide tours to prospective enrollees and their families/representatives
Monitor participant schedules in the PACE Center and ensure that participants are assisted to and from their center appointments on time
Assist Transportation Manager and department in producing and supervising daily transportation services, including participant transportation to and from the PACE center as well as transportation to appointments and activities in the community
Assist Recreation Therapist and department in managing the Participant Advisory Committee and attend all meetings
Assure participant attendance is tracked daily
Train new staff in Center procedures
Assure that all supervised staff members complete required competencies at new hire and annually
Complete 90-day reviews for all new hires and annual reviews

Qualifications

Bachelor's degree in human services field highly preferred
Minimum of one (1) year of experience with an older adult population required
Previous PACE experience a plus
Customer service background preferred
Management and leadership skills required
Experience with writing policies and procedures
Knowledge of OSHA safety requirements
Skilled in identifying and recommending problem resolution
Ability to work independently with minimum supervision
Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers
Must submit to and pass a pre-employment drug/alcohol screening and criminal background check
Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, Excel, Outlook, Word, and Electronic Health records
Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public
Must be free of communicable diseases that can be readily transmitted in the workplace
TB test is required within the first seven (7) days of employment. Hep B vaccinations are offered but not required
Operates in full compliance with all applicable federal, state, and local fire, health, safety, sanitation, emergency procedures, and other standards prescribed in law or regulations
