At Abode Care Partners, we are seeking a compassionate House Calls Nurse Practitioner to provide personalized medical care to seniors and patients with complex needs in various home settings. Responsibilities
  • We are looking for a highly motivated and experienced nurse practitioner who can plan, direct, and evaluate patient care to ensure the best possible outcomes.
  • The ideal candidate will have strong interpersonal communication skills, excellent time management skills, and the ability to work collaboratively with other healthcare professionals.
  • As a House Calls Nurse Practitioner, you will examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • You will also order and interpret laboratory and diagnostic testing as needed, collaborate with caregivers and other healthcare professionals, and ensure accurate medical records.
Requirements
  • An active state Nurse Practitioners license is required, along with current Nurse Practitioner National Board Certification and Active state DEA licensure.
  • A Master's degree in Nursing (MSN) is also necessary, as well as five or more years of nursing experience, with at least two years of nurse practitioner experience preferred.
  • Strong knowledge of HIPAA and OSHA regulations is essential, along with proficiency in medical instruments and equipment.
  • Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications is also required.

Salary Range:

$100,000 - $120,000 per year
