Job Title: Hospice LPN Position
Job Summary: We are seeking an experienced Licensed Practical Nurse (LPN) to join our hospice care team. The successful candidate will provide direct patient care, complete documentation, and coordinate services with our team.
Responsibilities:
- Provide direct patient care, including using various skills and coordinating services.
- Complete documentation as required by policy and procedure.
- Follow the plan of care developed by the Registered Nurse.
- Coordinate hospice services with our team and other agencies.
- Accept and transcribe physician's orders.
Requirements:
- Successful completion of nursing education from an approved school.
- Current LPN licensure in the state and one year of clinical nursing experience.
- Ability to independently organize work and provide effective health care services.
- Effective communication skills (oral and written).
- CPR certification.
