Job Title: Remote RN Care Coordinator

Anchor Health is seeking a skilled and compassionate Remote RN Care Coordinator to join our team.

About the Role:

The Remote RN Care Coordinator will be responsible for providing high-quality patient care and support to patients and their families in the comfort of their own homes. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and their families to determine their needs and develop individualized care plans.
  • Provide education and support to patients and their families on disease management, medication adherence, and other relevant topics.
  • Collaborate with healthcare providers and other members of the care team to ensure seamless care coordination.
  • Document patient interactions and care plans in a timely and accurate manner.
  • Participate in regular team meetings and contribute to the development of care plans and other initiatives.
Requirements:
  • Current RN license in California.
  • Minimum of one year of experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing.
  • Excellent observation, verbal and written communication skills, and nursing skills per competency checklist.
  • Valid CA driver license and an automobile that is insured in accordance with the requirements of the state of California and is in good working order.
