Mental Health Support Specialist with Community Focus

We are seeking a dedicated and compassionate Community-Based Case Manager to join our team at Zepf Center. As a vital member of our mental health services, you will play a crucial role in supporting adult clients who are coping with mental illness.
Job SummaryThe successful candidate will be responsible for providing extensive community support and coordinating services to ensure the needs of our clients are met. You will assist clients in developing short and long-term goals and help them acquire necessary community resources.
Key Responsibilities- Evaluate client strengths and requirements to develop a comprehensive care plan.
- Collaborate with clients and relevant team members to formulate a detailed treatment strategy.
- Maintain regular contact with clients to ensure stability and progress towards their goals.
- Link and refer clients to other services, programs, and agencies as needed.
- Assist clients in tracking and keeping appointments.
- Complete required paperwork and documentation in accordance with agency standards.
- High School Diploma or GED required with demonstrated mental health experience; or Bachelor's degree in social work or three years of mental health experience preferred.
- CDCA certification required for SUD (Substance Use Disorder).
- Must have a valid driver's license and acceptable driving record.
