About the Role

At Northeast Ohio Neighborhood, we are seeking a skilled Patient Centered Med Home Care Coordinator to join our team. This role plays a vital part in facilitating care coordination services for patients who require wellness and preventive care.

Key Responsibilities
  • Manage population health data repository, generating reports and patient profiles to inform care management decisions.
  • Collaborate with care teams to optimize patient follow-through with care plans.
  • Provide pre-visit and follow-up direction and support to patients, families, and healthcare providers.
Education and Qualifications

We require a High School Diploma or equivalent, and prefer a Bachelor's degree in Health or Social Sciences, Business, Health Care Administration, Public Health or Health Education.

Minimum Requirements
  • Excellent verbal and written communication skills, along with good listening skills.
  • Knowledge of health disparities and chronic disease management treatment resources.
  • Strong organizational skills, attention to detail, and timely documentation.
  • Proven critical thinking and problem-solving skills.
Technical Skills
  • Demonstrated knowledge and proficiency in Microsoft Office and Outlook.
  • Ability to become proficient in NextGen software.
Compensation and Benefits

We offer an estimated salary of $55,000 - $65,000 per annum, commensurate with experience. Additional benefits include opportunities for professional growth and development within our organization.
