Senior Outpatient Counseling Specialist

Job DescriptionAs a Senior Outpatient Counseling Specialist, you will play a vital role in delivering comprehensive care to patients in our outpatient medical office setting. Working collaboratively with our interdisciplinary team, you will provide counseling services, ongoing care management support, and expertise through assessment, planning, implementation, and evaluation of individual patient and practice needs.
This position requires a highly organized professional with great attention to detail, adaptability to frequent change, and compliance with regulatory and departmental guidelines and policies. You will work closely with medical providers, care management, and the entire clinical team to enhance the quality of patient management and satisfaction, promoting continuity of care and cost effectiveness.
Responsibilities- Assess and document bio-psychosocial needs and prepare a formal intervention plan to suit the patient's requirements.
- Evaluate and identify possible or actual patient/family crises, providing crisis intervention and emotional support, and creating and facilitating effective plans for resolution.
- Deliver information and short-term therapeutic counseling to patients and their families in relation to social, psychological, and family situations.
- Develop and maintain relationships with outside community health and social agencies, monitoring ongoing and new community resources to meet patient needs.
- Refer patients and families with appropriate community assistance resources.
- Obtain, interpret, and document necessary information regarding patients and families, including bio-psychosocial history factors, and communicate to the interdisciplinary care team.
To be successful in this role, you must have:
- Licensed Clinical Social Worker (LCSW) certification.
- A Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education.
- BLS certification.
Experience working in an outpatient/ambulatory clinical care setting is preferred.
